Folinic acid (also known as leucovorin) is scientifically validated for use in certain types of anemia, specifically megaloblastic anemia resulting from folic acid deficiency. Folinic acid is a biologically active form of folate that can bypass some of the metabolic steps required to utilize folic acid. This property is especially important for patients with defects in folic acid absorption or metabolism. Clinical studies and medical guidelines confirm that folinic acid is effective in treating megaloblastic anemia caused by folate deficiency, which is characterized by the presence of abnormally large and immature red blood cells due to impaired DNA synthesis. This is distinct from iron-deficiency anemia, where folinic acid is not indicated.
Folinic acid is also used as an adjunct in cancer chemotherapy (notably with methotrexate), to "rescue" normal cells from the toxic effects of antifolate drugs, and in this context, it can help prevent or mitigate chemotherapy-induced anemia. The World Health Organization and various clinical guidelines endorse the use of folinic acid in these settings. Its use in other forms of anemia (such as anemia of chronic disease or iron-deficiency anemia) is not supported by scientific evidence.
