Vitamin B6 (pyridoxal 5'-phosphate, or PLP) plays a critical role in heme biosynthesis, which is essential for the formation of hemoglobin in red blood cells. Pyridoxal 5'-phosphate serves as a coenzyme for the enzyme δ-aminolevulinic acid synthase, the first step in heme synthesis. Deficiency of vitamin B6 can therefore lead to sideroblastic anemia, a specific type of anemia characterized by the presence of ringed sideroblasts in the bone marrow due to impaired heme synthesis and iron utilization. Supplementation with vitamin B6 can correct anemia in cases where it is caused by a deficiency of this vitamin, particularly hereditary or acquired sideroblastic anemia responsive to pyridoxine. However, for the vast majority of anemia cases—such as iron-deficiency anemia, vitamin B12 deficiency anemia, or anemia of chronic disease—vitamin B6 supplementation is not effective unless there is a documented deficiency. The evidence for B6 supplementation in anemia treatment, therefore, is limited to specific, uncommon subtypes of anemia and is well-documented in these cases, but not in the broader context of general anemia. Clinical guidelines and hematology references recognize vitamin B6 as a treatment for pyridoxine-responsive sideroblastic anemia, but not for other types. Thus, there is scientific justification for its use, but the overall evidence is limited in scope, reflected in a moderate evidence rating.
