Arginine nitrate is used to support or treat angina based on scientific rationale. Angina pectoris is chest pain caused by reduced blood flow to the heart muscle, often due to narrowing of the coronary arteries. L-arginine, the amino acid component of arginine nitrate, serves as a substrate for nitric oxide synthase, which produces nitric oxide (NO). NO is a potent vasodilator, relaxing blood vessels and improving blood flow. This mechanism is the same principle behind the use of organic nitrates (such as nitroglycerin) in angina, as these compounds are also NO donors. Several clinical studies have shown that oral L-arginine supplementation can increase NO production and potentially improve symptoms in patients with angina, though results are mixed and the effects are generally modest compared to standard therapies like nitrates or beta-blockers.

As for nitrate (inorganic or as part of other compounds), dietary nitrates (from sources like beetroot) have also been shown to increase NO availability and may have a beneficial effect on vascular function. However, the specific compound "arginine nitrate" (a salt combining L-arginine and nitrate) has limited direct clinical research in the context of angina. The scientific justification is extrapolated from the well-established roles of its individual components in NO production and vasodilation. Thus, while the mechanistic foundation is strong and there is moderate supporting evidence for its constituents, high-quality, direct clinical trials of arginine nitrate itself for angina are lacking.
