Astragaloside IV is a primary active compound isolated from Astragalus membranaceus, a well-known traditional Chinese medicinal herb. While Astragalus has a long-standing historical use in traditional medicine for cardiovascular conditions, the use of purified astragaloside IV for angina is supported by early-stage scientific evidence rather than tradition. Several preclinical studies (mainly in animal models) indicate that astragaloside IV can exert cardioprotective effects through multiple mechanisms relevant to angina, such as improving myocardial energy metabolism, reducing oxidative stress, attenuating inflammation, and enhancing coronary blood flow. For example, animal studies have shown that astragaloside IV can ameliorate myocardial ischemia-reperfusion injury, reduce infarct size, and improve cardiac function. Some of these effects are thought to be mediated via modulation of the PI3K/Akt and MAPK signaling pathways.
However, direct clinical evidence in humans with angina is limited. Only a small number of clinical studies (often in China, and sometimes combining astragaloside IV with other interventions) suggest possible benefits in improving symptoms and some biochemical markers, but these trials are generally small and of low methodological quality. Therefore, while there is promising mechanistic and early clinical evidence, larger and more rigorous human trials are needed to establish its efficacy and safety for angina.
In summary, the use of astragaloside IV for angina is justified by scientific (preclinical and limited clinical) evidence, but the overall quality and quantity of the data merit a modest evidence rating.
