Rating (out of 5): 1

Coleus forskohlii root (forskolin) has a long history of use in traditional Ayurvedic medicine for various cardiovascular conditions, including angina pectoris. In Ayurveda, preparations containing forskohlii root were believed to support heart health, improve circulation, and relieve chest pain associated with angina. The traditional rationale was based on its perceived effects of vasodilation and enhancement of cardiac function. Scientifically, forskolin—the active compound in Coleus forskohlii—has been shown in laboratory and animal studies to activate the enzyme adenylate cyclase, which increases cyclic AMP levels and leads to vasodilation and reduced blood pressure. While these mechanisms could theoretically benefit angina by improving coronary blood flow and reducing cardiac workload, direct clinical evidence in humans is extremely limited. Only a handful of small-scale studies or case reports indirectly suggest potential benefit, and no large, well-designed clinical trials have established its safety or efficacy for angina specifically. As a result, while there is a traditional basis for its use in angina and some plausible pharmacological mechanisms, the overall evidence supporting forskohlii root for angina remains weak (rated 1 out of 5). Anyone considering its use should consult a healthcare provider, particularly given possible drug interactions and the lack of robust clinical data.
