Tanshinone is a group of bioactive compounds extracted from the roots of Salvia miltiorrhiza (Danshen), a traditional Chinese medicinal herb. Its use in treating angina (chest pain due to myocardial ischemia) is supported by both historical practice and increasing scientific evidence. Several preclinical and clinical studies indicate that tanshinones, especially Tanshinone IIA, have cardioprotective effects. Mechanisms proposed include anti-inflammatory, antioxidant, anti-apoptotic, and vasodilatory actions, as well as improvement of coronary microcirculation and inhibition of platelet aggregation.

Meta-analyses and randomized controlled trials (mostly from China) have reported that preparations containing tanshinones (often as part of Danshen extracts or Tanshinone IIA sodium sulfonate injections) can reduce the frequency and severity of anginal attacks, improve electrocardiogram (ECG) results, and enhance overall clinical efficacy when used as an adjunct to conventional therapy. However, many of these studies are limited by small sample sizes, short follow-up periods, and variable methodological quality. International guidelines do not recommend tanshinone as a first-line therapy, but Chinese guidelines and pharmacopeia recognize its adjunctive use.

In sum, while there is a traditional basis for its use, there is also moderate scientific evidence (level 3/5) supporting tanshinone’s adjunctive role in angina management, though further high-quality, large-scale trials are needed for broader acceptance.
