Andrographis paniculata is a medicinal plant traditionally used in Asia for treating infections and inflammatory conditions, often referred to as the "King of Bitters." Its application as an alternative or adjunct to antibiotics is primarily based on its reputed antimicrobial and immune-modulating properties. Several in vitro studies have demonstrated that extracts of Andrographis, and its main active compound andrographolide, possess antibacterial activity against a range of pathogens, including Staphylococcus aureus and Escherichia coli. However, these effects are generally modest compared to conventional antibiotics and have not been reliably reproduced in human clinical studies.

Clinically, Andrographis has been most studied for upper respiratory tract infections, where meta-analyses suggest some benefit in reducing symptom severity and duration, likely due to anti-inflammatory and immune-enhancing effects rather than direct antibacterial action. There is limited evidence supporting Andrographis as a true alternative to antibiotics in treating bacterial infections. Most clinical trials focus on symptomatic relief rather than bacteriological cure. Consequently, while Andrographis shows promise as an adjunct to antibiotics or as a supportive remedy in mild infections, it should not be considered a direct substitute for antibiotic therapy in established bacterial infections. More rigorous, large-scale clinical trials are needed to clarify its role and efficacy as an alternative to antibiotics.
