Plantain (Plantago spp.), especially Plantago major and Plantago lanceolata, has a long history of traditional use in various cultures as a remedy for wounds, infections, and inflammatory conditions. The leaves have been applied topically or brewed as teas to treat skin infections, minor wounds, and respiratory ailments, often in contexts where antibiotics were unavailable. The rationale for its traditional use as an “alternative to antibiotics” comes from its perceived antimicrobial, anti-inflammatory, and wound-healing properties. Several in vitro studies have demonstrated that plantain extracts possess mild antibacterial activity against some Gram-positive and Gram-negative bacteria, likely due to phytochemicals such as aucubin, baicalein, and other iridoid glycosides. However, the potency of these effects is considerably less than that of pharmaceutical antibiotics, and there are few, if any, robust human clinical trials confirming its efficacy as a substitute for antibiotics in treating systemic infections. Most available evidence is preclinical or anecdotal, meaning that while plantain has some weak antimicrobial properties, it is not a validated replacement for antibiotics. Its main value lies in minor topical infections or as a supportive measure in folk medicine traditions. Anyone with a serious infection should not rely on plantain in place of evidence-based antibiotic therapy.
