Red Sage (Salvia miltiorrhiza), also known as Danshen, is primarily used in traditional Chinese medicine for cardiovascular and circulatory health, rather than as a direct antibiotic or alternative to antibiotics. However, traditional formulations have sometimes included Red Sage to "clear heat and toxins," a concept loosely interpreted as addressing infections in traditional East Asian medicine. There is limited scientific investigation into its direct antibacterial or antibiotic-alternative effects. Some in vitro studies have shown that extracts of Red Sage possess mild antimicrobial activity against certain bacteria, but these effects are generally weak compared to standard antibiotics, and robust clinical evidence in humans is lacking.

Thus, the use of Red Sage as an alternative to antibiotics is primarily justified by traditional practice rather than strong scientific validation. Most research and traditional use focus on its anti-inflammatory, antioxidant, and circulatory benefits rather than direct pathogen eradication. Current evidence does not support substituting Red Sage for established antibiotic therapies in bacterial infections. More rigorous research is needed to determine any clinically relevant antimicrobial effects in humans.
