Lactoferrin, a multifunctional glycoprotein found in milk and other secretory fluids, has been investigated for its potential to mitigate some side effects of antibiotics, particularly antibiotic-associated diarrhea (AAD) and gut dysbiosis. The rationale is based on lactoferrin’s antimicrobial, prebiotic, and anti-inflammatory properties, which may help restore gut microbiota balance disrupted by antibiotics. Several small clinical trials, mostly in pediatric populations, have explored lactoferrin’s ability to reduce the incidence and severity of AAD. For example, studies in hospitalized children suggest that supplementation with bovine lactoferrin may decrease the risk of diarrhea during antibiotic therapy. The proposed mechanisms include inhibiting the growth of pathogenic bacteria (such as Clostridioides difficile), supporting the growth of beneficial bacteria, and modulating immune responses in the gut.

However, the overall evidence remains limited, with few high-quality randomized controlled trials in adults. Meta-analyses generally conclude that while lactoferrin shows promise, more robust and large-scale studies are needed to confirm efficacy, especially in diverse patient populations and for various antibiotics. There is also emerging interest in its ability to reduce the risk of secondary infections during antibiotic therapy, but this area is less well-studied. In summary, there is some scientific evidence to support lactoferrin’s use for alleviating certain side effects of antibiotics, but the strength of evidence is moderate at best (rated 2/5), and it should not be considered a substitute for well-established supportive measures such as probiotics.
