Pregnenolone is a neurosteroid precursor that has been the subject of investigation for its potential effects on mood and anxiety. There is some preliminary scientific evidence supporting its role in anxiety, but the data is limited and largely comes from small clinical trials and preclinical studies. Pregnenolone is thought to modulate the activity of neurotransmitter systems in the brain, including GABAergic and glutamatergic systems, both of which are involved in anxiety regulation. A small number of human studies have suggested pregnenolone may reduce anxiety symptoms, especially in populations with certain psychiatric disorders. For example, a 2010 randomized controlled trial found pregnenolone supplementation reduced anxiety in individuals with schizophrenia. Animal studies have shown that pregnenolone and its metabolites can decrease anxiety-like behavior, likely through modulation of GABAA receptors.

However, robust clinical trials in the general population with primary anxiety disorders are lacking. The current evidence does not reach the level of broad scientific consensus or clinical guideline endorsement. Thus, while there is a plausible biological rationale and some early supportive data, more high-quality research is needed before pregnenolone can be considered a validated treatment for anxiety.
