Hyperforin is a major active constituent of Hypericum perforatum (St. John's Wort), a traditional herbal remedy with a long history of use for mood disorders. Its primary established use is in the treatment of mild to moderate depression. Some clinical and preclinical studies have explored its effects on anxiety, but the evidence is limited and less robust compared to its antidepressant properties. Hyperforin is believed to exert its effects by inhibiting the reuptake of several neurotransmitters, including serotonin, dopamine, norepinephrine, GABA, and glutamate, which may theoretically influence anxiety symptoms. However, randomized controlled trials (RCTs) specifically assessing hyperforin or St. John's Wort for anxiety disorders are sparse. A few small studies and meta-analyses suggest some potential anxiolytic effects, but methodological issues and inconsistent results limit definitive conclusions. Major clinical guidelines do not currently recommend hyperforin or St. John's Wort as a first-line treatment for anxiety disorders due to insufficient high-quality evidence. Overall, while there is some scientific rationale and preliminary clinical data, the evidence supporting the use of hyperforin for anxiety disorders is weak to moderate at best.
