Lithium orotate is a salt containing lithium (a trace mineral) and orotic acid, often marketed as a dietary supplement. Its use for anxiety disorders is primarily based on the well-established psychiatric use of prescription lithium carbonate for bipolar disorder and, to a lesser extent, for mood stabilization. However, scientific evidence supporting lithium orotate itself as an effective treatment for anxiety disorders is minimal to nonexistent. Most available data on lithium’s therapeutic effects come from studies on prescription lithium salts (like lithium carbonate and lithium citrate) at much higher doses than those found in over-the-counter orotate supplements.

A few small, poorly designed studies and anecdotal reports suggest that low-dose lithium, as provided by lithium orotate, may have mood-stabilizing or anxiolytic effects. However, these claims are not substantiated by rigorous clinical trials. Moreover, lithium orotate is not approved by the FDA for any psychiatric condition, and there are safety concerns regarding its use without medical supervision, as lithium can have a narrow therapeutic window and potential for toxicity.

Most claims are extrapolated from data on pharmaceutical lithium salts, which are prescribed under strict medical supervision for specific mood disorders, not anxiety.
