SAMe (S-adenosyl-L-methionine) is a naturally occurring compound involved in methylation processes in the body, including neurotransmitter synthesis. Its use in psychiatry is best studied in depression, where several clinical trials and meta-analyses suggest modest antidepressant effects. Since anxiety frequently co-occurs with depression and shares some pathophysiological mechanisms, SAMe has been considered for anxiety disorders as well. However, the scientific evidence directly supporting SAMe’s efficacy for primary anxiety disorders (such as generalized anxiety disorder, panic disorder, or social anxiety disorder) is limited.

Most available studies focus on depressive symptoms, sometimes including secondary measures of anxiety. Some small trials and open-label studies have noted reductions in anxiety symptoms as a secondary outcome, but these findings are not robust or consistent enough to establish efficacy specifically for anxiety disorders. There are no large, high-quality randomized controlled trials (RCTs) directly examining SAMe’s effects on primary anxiety disorders alone. Some mechanistic rationale exists, as SAMe may modulate neurotransmitters (such as serotonin and dopamine) implicated in anxiety, but this remains theoretical.

In summary, while there is some scientific rationale and limited preliminary evidence for the use of SAMe in anxiety, the overall quality and quantity of evidence is low. The best-supported use for SAMe is as an adjunct in depression, with only weak and indirect support for anxiety disorders. More targeted research is needed before it can be recommended for anxiety with confidence.
