Tetrahydropalmatine (THP) is an isoquinoline alkaloid isolated from several plant species, including Corydalis yanhusuo, widely used in traditional Chinese medicine. In recent years, preclinical studies have explored its potential anxiolytic effects. Animal research has shown that THP exhibits anxiolytic-like activity in behavioral models, such as the elevated plus maze and open field test. The proposed mechanism involves modulation of dopaminergic and GABAergic neurotransmission, as THP is known to act as a dopamine receptor antagonist and may influence GABA receptors, both relevant to anxiety regulation. While these findings are promising, human clinical trials are extremely limited. A few small-scale studies and case reports suggest some benefit for anxiety and related symptoms, but robust, placebo-controlled clinical trials are lacking. Thus, although there is a scientific rationale for THP’s use in anxiety disorders, the evidence is primarily preclinical, and no major health authorities endorse its use for this indication. Most systematic reviews and expert opinions characterize the evidence as preliminary. In summary, there is some scientific basis for exploring THP in the treatment of anxiety disorders, largely supported by animal data and mechanistic plausibility. However, the current clinical evidence is weak and insufficient to justify routine use in humans for anxiety disorders.
