Puerarin is an isoflavone compound extracted from the root of Pueraria lobata (Kudzu), a herb frequently used in traditional Chinese medicine. Its cardiovascular effects have been of interest in modern biomedical research, particularly in the context of arrhythmia. Several preclinical studies (in vitro and animal models) have demonstrated that puerarin can modulate cardiac electrophysiology, including prolonging the action potential duration and refractory period, and inhibiting abnormal calcium influx. Such actions may theoretically reduce susceptibility to arrhythmias. Additionally, puerarin has been shown to possess antioxidant properties and to improve coronary blood flow, which are potentially beneficial in arrhythmic conditions secondary to ischemia. A handful of small-scale clinical studies and case reports from China suggest that puerarin injections may help reduce arrhythmic episodes, particularly in patients with ischemic heart disease. However, the methodological quality of these studies is generally low, with limitations such as small sample sizes, lack of rigorous controls, and potential publication bias. There is no robust, large-scale, placebo-controlled clinical trial evidence to firmly establish puerarin’s efficacy and safety for arrhythmia in humans. In summary, while there is some preclinical and preliminary clinical evidence suggesting a potential anti-arrhythmic effect of puerarin, the overall strength of scientific evidence remains low (rated 2/5), and more high-quality clinical trials are needed to validate its use for this indication.
