Hydroxytyrosol is a phenolic compound predominantly found in olives and olive oil. Its use to support arterial and cardiovascular health is justified by a growing body of scientific evidence, though the majority comes from preclinical studies and small-scale clinical trials rather than large, definitive randomized controlled trials. Hydroxytyrosol exhibits potent antioxidant and anti-inflammatory properties, which are believed to protect the endothelium (the inner lining of blood vessels) from oxidative stress and inflammation—both key contributors to atherosclerosis and arterial dysfunction.

Several in vitro and animal studies have demonstrated that hydroxytyrosol can reduce oxidative damage to LDL cholesterol, a key driver of plaque formation in arteries. Human clinical evidence, while limited, includes studies showing that consumption of olive oil rich in hydroxytyrosol leads to improved markers of endothelial function and reduced biomarkers of oxidative stress. The European Food Safety Authority (EFSA) has acknowledged that hydroxytyrosol and its derivatives contribute to the protection of blood lipids from oxidative damage when olive oil contains at least 5 mg of hydroxytyrosol and its derivatives per 20 g of oil.

In summary, while more large-scale human trials are needed, the current scientific evidence supports the role of hydroxytyrosol in supporting arterial health, primarily via its antioxidant effects. Thus, its use is scientifically justified, but the strength of evidence is moderate (rated 3/5).
