Inositol nicotinate (also known as inositol hexanicotinate) has traditionally been used in some countries, particularly in Europe, to support vascular health and improve peripheral circulation. Its use for arteriosclerosis is based more on traditional and empirical practices rather than robust scientific evidence. The rationale stems from the known vasodilatory and lipid-modifying properties of nicotinic acid (niacin), from which inositol nicotinate is derived. Early clinical studies suggested potential benefits in improving blood flow in conditions such as intermittent claudication, which is often associated with peripheral arterial disease (a manifestation of arteriosclerosis). However, these studies were generally small, of limited duration, and often methodologically weak.

There is a lack of high-quality, large-scale randomized controlled trials directly evaluating inositol nicotinate's efficacy in treating or preventing arteriosclerosis itself. Most modern clinical guidelines do not recommend inositol nicotinate for arteriosclerosis due to insufficient evidence.
