Marine oil, particularly fish oil rich in omega-3 polyunsaturated fatty acids (PUFAs) such as EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), is widely used to support and manage symptoms of arthritis, especially rheumatoid arthritis (RA). Multiple randomized controlled trials and meta-analyses have provided evidence that supplementation with marine oil can reduce joint pain, morning stiffness, and the number of tender joints in individuals with RA. The anti-inflammatory properties of omega-3 fatty acids are thought to arise from their ability to modulate the production of pro-inflammatory cytokines and eicosanoids, resulting in decreased inflammation and immune activity in the joints.
A 2017 Cochrane review concluded that marine oil supplementation results in a small but statistically significant improvement in patient-reported pain and joint function in RA. Other systematic reviews have supported these findings, although the magnitude of benefit is generally considered modest. There is less robust evidence for benefits in osteoarthritis, but some studies suggest a potential reduction in symptoms.
Overall, the use of marine oil for arthritis is justified by a moderate body of scientific evidence, especially for autoimmune inflammatory arthritis like RA. Clinical guidelines from several rheumatology organizations acknowledge marine oil as an adjunctive therapy for arthritis management, primarily to help reduce pain and inflammation. However, marine oil is not considered a replacement for disease-modifying anti-rheumatic drugs (DMARDs) or other standard treatments.
