D-ribose is a naturally occurring sugar and a key component of adenosine triphosphate (ATP), the molecule responsible for storing and transferring energy in cells. Its supplementation has been investigated for its potential to improve athletic performance and aid exercise recovery due to its role in cellular energy production. Some small clinical studies have indicated that D-ribose supplementation might help restore ATP levels more rapidly in muscle following high-intensity exercise, potentially reducing muscle soreness and fatigue. For example, a few pilot studies in the early 2000s suggested that D-ribose might improve exercise capacity and recovery, particularly in individuals with compromised energy metabolism, such as those with heart disease or chronic fatigue syndrome.

However, the evidence supporting D-ribose as an effective athletic or exercise aid in healthy individuals is limited and inconsistent. Larger, well-controlled studies are lacking, and reviews by reputable sources such as the Natural Medicines Comprehensive Database rate the evidence as insufficient or only possibly effective. There is little to no evidence supporting significant performance enhancement in trained athletes. Safety profiles appear acceptable for short-term use, but the long-term effects are unknown. In summary, while there is a plausible biochemical basis and some weak scientific evidence for D-ribose supporting exercise recovery, the overall level of clinical support is low, and its benefits for athletic performance are unproven.
