Specialized pro-resolving mediators (SPMs) are a class of lipid-derived molecules, including resolvins, protectins, and maresins, which are enzymatically generated during the resolution phase of inflammation. There is a growing body of preclinical research indicating that SPMs play a key role in actively resolving inflammation, rather than simply suppressing it. In autoimmune disorders, where chronic inflammation and failure to resolve immune responses underlies tissue damage, SPMs have been investigated as potential modulators of immune activity.

Animal studies and in vitro experiments have shown that SPMs can reduce the production of pro-inflammatory cytokines, promote the clearance of apoptotic cells, and restore tissue homeostasis. For example, SPMs have demonstrated beneficial effects in experimental models of rheumatoid arthritis, multiple sclerosis, and systemic lupus erythematosus by reducing inflammatory cell infiltration and ameliorating disease severity.

However, clinical evidence in humans remains limited. Only a few small pilot trials and observational studies have explored SPM supplementation in autoimmune conditions, with mixed and preliminary results. No large-scale randomized controlled trials have yet established clear efficacy or optimal dosing for SPMs in treating autoimmune diseases. Thus, while the scientific rationale is promising and supported by mechanistic and animal data, robust clinical validation is still lacking, resulting in a moderate (2/5) evidence rating for their use in autoimmune disorders at this time.
