Beta-sitosterol is a plant-derived sterol that has been investigated for its potential benefits in managing symptoms of Benign Prostatic Hyperplasia (BPH), a non-cancerous enlargement of the prostate gland common in older men. Several randomized, placebo-controlled trials and meta-analyses have evaluated beta-sitosterol's efficacy in this context. Notably, a 1999 meta-analysis published in the British Journal of Urology reviewed four high-quality randomized controlled trials and found that beta-sitosterol improved urinary symptom scores and flow measures significantly more than placebo. The improvements were similar to, though generally less robust than, those seen with conventional pharmaceuticals like finasteride or alpha-blockers.
The proposed mechanisms for beta-sitosterol's effects include inhibition of 5-alpha-reductase (the enzyme converting testosterone to dihydrotestosterone, which drives prostate growth) and anti-inflammatory actions. However, beta-sitosterol does not appear to reduce prostate size; rather, it alleviates lower urinary tract symptoms (LUTS), such as urinary frequency, urgency, and flow difficulties.
Despite these positive findings, the overall quality and duration of studies are limited, and most trials have been relatively short-term (typically less than 6 months). There is also a lack of large-scale, long-term studies confirming durability and safety. As a result, while there is meaningful scientific evidence supporting the use of beta-sitosterol for symptomatic relief in BPH, it is considered moderate and insufficient to recommend over standard medical therapies.
