Nettle (Urtica dioica), specifically the root extract, has been used in traditional medicine for urinary issues, including benign prostatic hyperplasia (BPH). Scientific interest has led to several clinical studies and reviews investigating its efficacy in BPH management. While the exact mechanism is not fully understood, nettle root is thought to influence sex hormone-binding globulin and inhibit certain enzymes involved in prostate cell proliferation, potentially reducing prostate size and urinary symptoms.

A number of randomized controlled trials (RCTs) and meta-analyses suggest that nettle root extract can improve lower urinary tract symptoms associated with BPH, such as nocturia and urinary flow, when compared to placebo. Some studies have also found nettle root to be effective when used in combination with other phytotherapeutics like saw palmetto. However, these effects appear modest and may not be as pronounced as those of standard pharmaceutical treatments (e.g., alpha blockers, 5-alpha-reductase inhibitors).

The European Medicines Agency acknowledges traditional use of nettle root for BPH-related symptoms, and its use is included in some European herbal monographs. Nevertheless, the overall quality of evidence is moderate, with some studies limited by small sample sizes, short durations, and varying product formulations. As such, while nettle root's use for BPH is supported by both tradition and a moderate body of scientific evidence, more rigorous research is needed to establish its efficacy and safety profile.
