Several randomized, placebo-controlled clinical trials and systematic reviews have indicated that beta-sitosterol supplementation can help improve urinary symptoms such as frequency, urgency, and flow rate in men with mild to moderate BPH. These effects are likely due to its anti-inflammatory properties and its ability to inhibit 5-alpha-reductase, an enzyme involved in the metabolism of testosterone to dihydrotestosterone, which plays a role in prostate enlargement.

There is less direct evidence for beta-sitosterol supporting general bladder health in women or in individuals without BPH, and its use in this context is less well established. However, because prostate enlargement can impact bladder function by causing urinary retention or incomplete bladder emptying, beta-sitosterol's benefit for bladder symptoms in men with BPH is scientifically supported.
