Glycosaminoglycans (GAGs), such as chondroitin sulfate and hyaluronic acid, play a significant role in the structure and function of the bladder lining. The bladder’s mucosal surface is coated with a layer of GAGs, which act as a protective barrier, preventing irritating substances in urine from penetrating and causing inflammation of the underlying tissues. Clinical evidence supports the use of GAG replenishment therapy, particularly in conditions like interstitial cystitis/bladder pain syndrome (IC/BPS). Several randomized controlled trials and systematic reviews have examined intravesical (directly into the bladder) administration of GAGs, especially hyaluronic acid and chondroitin sulfate, reporting improvements in pain, urinary frequency, and urgency in some patients. However, the quality of evidence varies, with some studies showing modest or inconsistent benefits, and long-term efficacy remains an area of ongoing research. Oral GAG supplementation has less robust evidence compared to intravesical administration. Overall, while not universally effective for all bladder conditions, scientific studies provide a moderate level of support for the use of glycosaminoglycans as a therapeutic option for certain bladder disorders, particularly those involving defects in the bladder’s protective lining.
