D-mannose is a type of sugar that has gained popularity as a supplement for preventing and managing urinary tract infections (UTIs), particularly bladder infections (cystitis) caused by Escherichia coli (E. coli). The scientific rationale lies in the way E. coli bacteria attach to the uroepithelial cells lining the bladder. These bacteria use hair-like appendages called fimbriae to bind to mannose residues on the bladder wall. By supplementing with D-mannose, it is believed that free mannose in the urine acts as a decoy, binding to the bacteria and preventing them from adhering to the bladder lining, thus facilitating their elimination through urination.
Several randomized controlled trials and systematic reviews have evaluated the efficacy of D-mannose for preventing recurrent UTIs. Some studies report that D-mannose supplementation may significantly reduce the risk of recurrent infections, with efficacy comparable to some antibiotics and fewer side effects. For example, a 2014 study in World Journal of Urology and a 2023 Cochrane review both suggest that D-mannose may reduce UTI recurrence, though larger, higher-quality trials are still needed. The evidence for using D-mannose for acute treatment is more limited, but some small studies report symptom improvement.
Overall, while not yet standard clinical practice, the use of D-mannose for UTI prevention is supported by moderate scientific evidence and is considered safe for most individuals, although more robust research is warranted to clarify its efficacy and optimal dosing.
