Lumbrokinase is a group of proteolytic enzymes extracted from earthworms (notably Eisenia fetida) that has demonstrated fibrinolytic (clot-dissolving) capabilities. The use of earthworm extracts in traditional medicine, particularly in East Asia, dates back centuries, but modern research has focused on the purified enzyme complex known as lumbrokinase. Several in vitro and animal studies have shown that lumbrokinase can enhance fibrinolysis by directly degrading fibrin and activating plasminogen, mechanisms similar to tissue plasminogen activator (tPA).
Small clinical trials, mostly conducted in China, suggest that oral lumbrokinase may reduce blood viscosity and improve markers of fibrinolytic activity in patients at risk for cardiovascular and cerebrovascular events. A few randomized controlled trials report benefits in secondary stroke prevention and in patients with coronary artery disease, but these studies are generally small and sometimes lack rigorous methodology or blinding. Systematic reviews highlight the potential, but consistently emphasize the need for larger, high-quality trials to confirm efficacy and safety, particularly in comparison to standard anticoagulants.
Overall, scientific evidence supporting the use of lumbrokinase for the prevention of blood clots is promising but not definitive. It is recognized as a fibrinolytic agent in China and some other countries, but is not approved by regulatory agencies such as the FDA for this indication. Users should be cautious and consult healthcare professionals before considering it as an alternative to established therapies.
