Tanshinone is a group of diterpene quinone compounds primarily extracted from the root of Salvia miltiorrhiza (Danshen), a traditional Chinese medicinal herb. There is moderate scientific evidence supporting the use of tanshinones in the prevention of blood clots. Several in vitro and in vivo studies have demonstrated that tanshinone IIA, the most studied of these compounds, exhibits antithrombotic and anticoagulant effects. The mechanisms proposed include inhibition of platelet aggregation, suppression of thromboxane formation, and modulation of the coagulation cascade. Preclinical studies in animal models have shown that tanshinone IIA can reduce thrombus formation and improve blood flow in arteries subjected to injury or occlusion. Limited clinical studies, mostly from China, have suggested a potential benefit for Danshen extracts (which contain tanshinones) in improving microcirculation and reducing the risk of thrombosis, especially in patients with cardiovascular disease. However, these clinical studies often use whole herb extracts rather than purified tanshinones, and the methodological quality is variable. No large-scale, high-quality randomized controlled trials of purified tanshinone for blood clot prevention exist. As a result, while there is a scientific rationale and some supporting evidence from laboratory and small clinical studies, definitive conclusions about efficacy in humans are still lacking. More rigorous clinical research is needed to firmly establish tanshinone’s role in blood clot prevention.
