Allium species, particularly garlic (Allium sativum) and onion (Allium cepa), have a long history of use in traditional medicine for the treatment of skin infections and boils. In various traditional healing systems, crushed garlic or onion is applied topically to boils with the belief that their natural antimicrobial and anti-inflammatory properties help to draw out pus and promote healing. Historical references to such practices can be found in Ayurvedic, Unani, and folk European medicine. The rationale for their use lies in the presence of organosulfur compounds (such as allicin in garlic), which exhibit antimicrobial activity in laboratory settings. However, while laboratory studies show that garlic extracts can inhibit the growth of some bacteria, including Staphylococcus aureus (a common cause of boils), robust clinical trials or systematic studies on the effectiveness of Allium preparations specifically for the treatment of boils in humans are lacking. Therefore, the use of Allium for boils is primarily supported by traditional use rather than scientific validation, with only limited in vitro evidence suggesting a plausible mechanism.
