Carthamus, commonly known as safflower, has a long history of use in traditional Chinese medicine (TCM) and other herbal systems for promoting blood circulation, dispelling blood stasis, and alleviating pain. In TCM, Carthamus tinctorius (the most commonly referenced species) is often included in formulas for treating bruises, contusions, and other trauma-related conditions, with the rationale that it helps prevent and resolve subcutaneous blood accumulation. However, while its traditional application for bruises is well-documented in classical herbal texts and widely practiced, the scientific evidence supporting its efficacy for bruising prevention or treatment is sparse. A limited number of animal and in vitro studies suggest potential anti-inflammatory and anticoagulant effects, but robust clinical trials in humans specifically addressing bruising are lacking. Thus, the use of Carthamus for bruises is primarily justified by traditional use, with low to moderate supporting evidence. Its inclusion in multi-herb trauma formulas is common, but evidence from controlled clinical trials is inadequate to definitively validate its efficacy for this indication.
