Pyridoxine (Vitamin B6) has been traditionally used in the management of symptoms such as burning feet or hands, particularly in the context of neuropathies. The historical basis for this practice is rooted in observations from the early to mid-20th century, when nutritional deficiencies—especially in B vitamins—were recognized as causes of peripheral neuropathy. Classic "burning feet syndrome" was notably observed in populations with malnutrition, such as prisoners of war and individuals with severely restricted diets, and was linked to deficiencies in B vitamins including B6. Supplementation with pyridoxine, often alongside other B vitamins, was reported to alleviate symptoms in these cases. However, scientific validation for pyridoxine supplementation in burning feet or hands outside the context of an established deficiency is limited. There is clear evidence that severe B6 deficiency can cause peripheral neuropathy with burning sensations, and supplementation in these cases is effective. On the other hand, most modern cases of burning feet or hands are due to diabetes, nerve compression, or other causes unrelated to vitamin B6 deficiency. High doses of pyridoxine can, in fact, cause neuropathy, making indiscriminate supplementation potentially harmful. Thus, while the traditional use is well-documented, current evidence supports B6 supplementation only when deficiency is confirmed or strongly suspected.
