Lactoperoxidase is an enzyme naturally present in saliva with antimicrobial properties. Its use in oral health products, including those targeting canker sores (aphthous ulcers), is based on its ability to generate hypothiocyanite ions, which inhibit the growth of various bacteria and potentially reduce oral inflammation. Several studies have explored the efficacy of lactoperoxidase-containing oral rinses or gels in managing oral mucosal conditions. For example, some randomized controlled trials have shown that lactoperoxidase, often combined with other enzymes like glucose oxidase and lysozyme, may help reduce the discomfort and duration of canker sores by supporting the natural antimicrobial defense mechanisms of saliva and modulating the oral microbiome.
However, the body of evidence specifically addressing canker sores is limited. Most studies focus on general improvements in oral hygiene or reductions in plaque and gingivitis, rather than on aphthous ulcers specifically. A few small clinical trials and case reports suggest a benefit for canker sore healing, but these findings are not yet robust or widely replicated. Thus, the scientific support for lactoperoxidase in the treatment of canker sores exists but is relatively modest, warranting a rating of 2 out of 5.
