Escin (sometimes spelled "Escein") is a natural mixture of saponins derived from the seeds of the horse chestnut tree (Aesculus hippocastanum). It is widely used to support capillary health, particularly in the management of chronic venous insufficiency (CVI) and related conditions such as varicose veins and edema. The scientific rationale for its use is based on both pharmacological studies and clinical trials indicating that escin exerts anti-inflammatory, vasoprotective, and edema-reducing effects. Mechanistically, escin is thought to increase venous tone, decrease capillary permeability, and improve lymphatic drainage, thereby reducing swelling and supporting capillary integrity.

Multiple randomized controlled trials and meta-analyses have found that standardized horse chestnut seed extracts containing escin are effective in reducing leg pain, swelling, and other symptoms of venous insufficiency compared to placebo. For example, a 2012 Cochrane review concluded that horse chestnut seed extract is more effective than placebo for short-term treatment of CVI symptoms, with a safety profile comparable to placebo. However, the quality of some studies varies, and more high-quality, long-term trials are still needed. Nonetheless, the overall body of evidence supports the use of escin for supporting capillary and venous health, justifying a relatively high evidence rating.
