Lignans are a group of polyphenolic compounds found in plants, especially in seeds such as flaxseed, as well as in whole grains, legumes, and some vegetables. Their use in supporting cardiovascular health is primarily supported by scientific evidence, though much of it is observational or based on animal and in vitro studies rather than large-scale randomized controlled trials in humans. Lignans are metabolized by gut bacteria into enterolignans, such as enterodiol and enterolactone, which have been associated with beneficial effects on cardiovascular risk factors.
Several epidemiological studies have found that higher dietary intake of lignans—particularly from flaxseed—is associated with improvements in lipid profiles, such as reductions in total cholesterol and LDL cholesterol. Some clinical trials have shown that flaxseed supplementation may modestly lower blood pressure and improve endothelial function, both of which are important for cardiovascular health. The proposed mechanisms include lignans’ antioxidant, anti-inflammatory, and estrogenic activities, which may help reduce atherosclerosis and improve blood vessel function.
However, the evidence is not uniformly strong or consistent across all studies. The magnitude of benefit appears modest, and the studies often use flaxseed (which contains multiple bioactive components) rather than isolated lignans. Overall, while there is plausible scientific rationale and some supportive data, the use of lignans for cardiovascular disease prevention or treatment should be considered as adjunctive to established therapies, pending further research.
