S-allylcysteine and Cardiovascular Disease: Evidence Overview

S-allylcysteine (SAC) is a water-soluble organosulfur compound derived from aged garlic extract. There is a moderate body of scientific evidence supporting its use as an adjunct for cardiovascular health. Preclinical studies and several small clinical trials have indicated that SAC may exert beneficial effects through antioxidant, anti-inflammatory, and lipid-lowering properties.

Notably, research has shown that SAC can reduce oxidative stress and inhibit low-density lipoprotein (LDL) oxidation, both of which are key factors in the development of atherosclerosis. Some animal studies have demonstrated that SAC supplementation can reduce blood pressure and improve endothelial function. In humans, small randomized controlled trials suggest that aged garlic extract (which contains SAC) may lower blood pressure, improve arterial stiffness, and modestly reduce cholesterol levels; however, these studies often use the whole extract rather than isolated SAC, making it difficult to attribute effects solely to this compound.

Despite promising results, large-scale, long-term clinical trials specifically investigating SAC's effects on cardiovascular outcomes are lacking. Most of the evidence comes from surrogate markers or from studies of aged garlic extract as a whole. Therefore, while the scientific rationale for SAC's use in cardiovascular disease is supported by mechanistic and early clinical data, more robust evidence is needed to establish its efficacy definitively.
