Vitamin B6 (pyridoxine) has been traditionally used to support or treat Carpal Tunnel Syndrome (CTS), particularly in the 1970s and 1980s, based on early reports suggesting that deficiency might contribute to nerve dysfunction. Initial small studies and anecdotal evidence promoted B6 supplementation for CTS, especially in individuals with marginal vitamin B6 status. However, subsequent larger and more rigorous clinical trials have failed to provide convincing evidence for its efficacy in treating CTS. Most recent reviews and guidelines do not recommend B6 supplementation as a standard therapy for CTS, except in rare cases of confirmed deficiency. The rationale for its traditional use stems from B6's role in nerve health and neurotransmitter synthesis, but the majority of people with CTS do not have vitamin B6 deficiency. Although vitamin B6 is generally safe at recommended doses, high doses can cause neuropathy and should be avoided. In summary, the use of vitamin B6 for CTS is primarily based on tradition and older, limited studies, with little scientific support from modern clinical trials.
