Vitamin B6 (pyridoxamine HCl) has been used traditionally for carpal tunnel syndrome (CTS) based on anecdotal reports and early small studies, particularly from the 1970s and 1980s. The rationale was that B6 deficiency might contribute to nerve dysfunction and that supplementation could alleviate symptoms like numbness and tingling. Some early observational studies and case reports suggested improvement in CTS symptoms with B6 supplementation, but these studies had significant methodological flaws, such as lack of control groups and small sample sizes.

Subsequent, more rigorous clinical trials and reviews have not found consistent evidence to support the efficacy of B6 supplementation in CTS, except in individuals with an established B6 deficiency (which is rare). Major guidelines and systematic reviews, including those from the American Academy of Orthopaedic Surgeons, do not recommend vitamin B6 for CTS in the absence of deficiency.

In summary, while there is a historical tradition of using vitamin B6 for CTS, scientific evidence supporting its use is weak to nonexistent except in cases of deficiency. Its continued use for CTS in the general population is not supported by high-quality clinical evidence.
