D-alpha tocopherol, the most biologically active form of vitamin E, is sometimes used to support eye health, including the prevention or slowing of cataract formation. The rationale for its use is grounded in its antioxidant properties, as oxidative stress is believed to play a significant role in the development of cataracts. Laboratory and animal studies have shown that vitamin E can help protect lens proteins and lipids from oxidative damage. In humans, observational studies have suggested that people with higher dietary intake or blood levels of vitamin E may have a lower risk of developing cataracts. However, randomized controlled trials (RCTs), including large-scale studies like the Age-Related Eye Disease Study (AREDS and AREDS2), have found limited or no significant benefit of vitamin E supplementation alone in preventing or treating cataracts. Some studies have tested combinations of antioxidants (including vitamin E), but the evidence for a clear protective effect remains inconclusive. Overall, while there is a plausible scientific rationale and some epidemiological support, direct evidence from interventional trials is weak, and vitamin E supplementation is not widely recommended as a primary therapy for cataracts. Its use is thus supported by scientific reasoning, but the level of evidence is low, meriting a rating of 2 out of 5.
