Alpha-Ketoglutarate (AKG) is occasionally used as an adjunct in cancer therapy, particularly to help reduce some of the side effects associated with chemotherapy, such as gastrointestinal mucositis. The justification for its use is based on limited scientific evidence, primarily from small clinical studies and animal research. Some studies suggest that AKG, due to its role as an intermediate in the Krebs cycle and its involvement in amino acid metabolism, may help maintain nitrogen balance, support protein synthesis, and improve intestinal mucosal integrity. For example, a few clinical trials have investigated oral AKG supplementation in cancer patients undergoing chemotherapy and reported a reduction in the severity of mucositis and diarrhea, as well as improved nutritional status and body weight maintenance. However, the evidence is not robust. Most studies are small, open-label, or lack rigorous controls, and systematic reviews highlight the need for larger, well-designed randomized controlled trials. The proposed mechanisms include AKG’s ability to act as an energy substrate for rapidly dividing cells (such as those in the gut lining), its anti-inflammatory effects, and its support of glutamine and arginine synthesis, both of which are important for gut health and immune function. In summary, there is some scientific evidence—albeit limited and preliminary—supporting the use of AKG for reducing certain chemotherapy-induced side effects. It is not a standard or widely accepted intervention, and more high-quality research is needed to establish efficacy and safety.
