Bifidobacterium (unspecified species) has been investigated for its potential to reduce chemotherapy-induced side effects, particularly gastrointestinal symptoms such as diarrhea, mucositis, and dysbiosis. Several randomized controlled trials and meta-analyses suggest that supplementation with Bifidobacterium-containing probiotics can help maintain gut microbiota balance during chemotherapy, which is often disrupted by cytotoxic drugs. This disruption is associated with increased intestinal permeability, inflammation, and a higher risk of infection.

For example, studies in cancer patients, especially those with colorectal and gastrointestinal cancers, have shown that probiotic formulas containing Bifidobacterium (often in combination with Lactobacillus species) can reduce the incidence and severity of diarrhea and may decrease the frequency of oral mucositis. A 2019 meta-analysis in Supportive Care in Cancer found that probiotics, including Bifidobacterium, significantly reduced the risk of severe diarrhea in patients receiving chemotherapy and/or radiotherapy. Mechanistically, Bifidobacterium species may modulate immune responses, strengthen the gut barrier, and inhibit pathogenic bacteria, contributing to improved gut health during cancer treatment.

However, the evidence is moderate rather than strong due to heterogeneity in probiotic strains, dosages, and study designs, as well as a lack of large, high-quality trials using Bifidobacterium alone. Therefore, while there is scientific support for Bifidobacterium's use in mitigating some chemotherapy side effects, more research is needed to determine optimal strains and protocols.
