Lentinula edodes (shiitake mushroom) mycelia, especially in the form of its extract known as lentinan, is supported by scientific evidence for use as an adjunct therapy in cancer patients undergoing chemotherapy. Several clinical studies, particularly from Japan and China, have shown that lentinan can enhance immune function and may help reduce some side effects associated with chemotherapy, such as immunosuppression, fatigue, and susceptibility to infections. Lentinan is a β-glucan polysaccharide that acts as a biological response modifier, stimulating various components of the immune system including macrophages, T cells, and natural killer cells.

Multiple randomized controlled trials and meta-analyses have suggested that patients receiving lentinan in combination with standard chemotherapy may experience improved quality of life, better maintenance of white blood cell counts, and in some cases, improved survival rates. Lentinan has been approved as an adjuvant cancer therapy in Japan for several decades, underscoring both its clinical use and the regulatory recognition of its benefits. While lentinan does not directly treat cancer or replace chemotherapy, its immunomodulatory effects are considered beneficial in supporting patients through challenging treatment regimens. However, while evidence is promising, variation in study quality and design means further large-scale, high-quality studies are warranted.
