N-Acetyl Cysteine (NAC) is supported by scientific evidence for use in reducing certain side effects of chemotherapy, particularly chemotherapy-induced toxicity such as nephrotoxicity from cisplatin and hemorrhagic cystitis from cyclophosphamide or ifosfamide. NAC is a precursor to glutathione, a major intracellular antioxidant, and works by replenishing glutathione stores and directly scavenging free radicals. This antioxidant property is believed to help mitigate the oxidative stress and cellular damage induced by some chemotherapeutic agents.
Clinical studies and animal models have shown that NAC can reduce the incidence and severity of organ toxicities associated with chemotherapy. For example, small randomized controlled trials and observational studies suggest that NAC may lower the risk of kidney damage in patients receiving cisplatin without significantly interfering with the antitumor efficacy of the drug. Similarly, in cases of hemorrhagic cystitis, NAC has demonstrated protective effects in preclinical and early clinical studies by reducing bladder inflammation and oxidative damage.
However, large-scale, high-quality clinical trials are limited, and routine use of NAC as a chemoprotective agent is not yet standard practice. Its use is sometimes considered in specific situations or in clinical trials, especially in patients at high risk for severe toxicity. More research is required to establish optimal dosing, timing, and to confirm its safety and efficacy across different chemotherapy regimens.
