Superoxide dismutase (SOD) is an enzyme with potent antioxidant activity, catalyzing the dismutation of superoxide radicals into oxygen and hydrogen peroxide. Its use as a supportive therapy during chemotherapy is primarily justified by scientific rationale rather than longstanding tradition. Chemotherapy can induce oxidative stress and generate reactive oxygen species (ROS), contributing to both its anti-cancer effects and the collateral damage to healthy tissues, which results in side effects such as mucositis, cardiotoxicity, and neurotoxicity. Preclinical studies (in vitro and animal models) have demonstrated that SOD and SOD mimetics can reduce oxidative tissue injury and inflammation caused by certain chemotherapeutic agents. Some small clinical trials and pilot studies have evaluated SOD or SOD mimetics (such as orgotein) to mitigate chemotherapy-induced side effects, particularly mucositis and soft tissue toxicity, with mixed results. However, the available clinical evidence is limited in scale and quality, and there remains concern that systemic antioxidant therapy could potentially interfere with the efficacy of chemotherapy by protecting cancer cells as well as normal tissues from ROS-mediated damage. In summary, while there is a sound biochemical basis and some early-stage evidence supporting the use of SOD for reducing chemotherapy side effects, large-scale, high-quality clinical trials are lacking. Therefore, the overall scientific validation is weak (rated 2/5), and routine clinical use is not widely established or endorsed in oncology guidelines.
