Uridine monophosphate (UMP) has scientific support for use in reducing certain side effects of chemotherapy, particularly those caused by the fluoropyrimidine class of drugs (such as 5-fluorouracil [5-FU] and capecitabine). Fluoropyrimidines can cause severe toxicity, including hand-foot syndrome and mucositis, due to their interference with RNA and DNA synthesis in healthy cells. UMP acts as a "rescue" agent because it can compete with toxic metabolites of 5-FU and replenish normal pyrimidine pools, thereby reducing cytotoxicity in non-cancerous tissues.
The best evidence comes from the use of uridine triacetate (a prodrug of uridine), which is FDA-approved as an antidote for 5-FU and capecitabine overdose or severe toxicity. Randomized controlled trials and case series have demonstrated that uridine triacetate administered within 96 hours of overdose or severe toxicity can significantly reduce mortality and morbidity. For example, a pivotal study published in JAMA (2016) reported a survival rate of 96% in patients treated with uridine triacetate compared to historical controls.
However, outside of emergency use for overdose or life-threatening toxicity, the evidence for routine use of UMP or uridine supplementation to mitigate milder side effects is less robust, and its use in this context is not standard. There is some preclinical and small-scale clinical evidence supporting its protective effects, but more research is needed.
In summary, uridine (especially as uridine triacetate) is scientifically validated as a rescue agent for severe fluoropyrimidine toxicity, with moderate-to-strong evidence supporting this specific use.
