Higenamine is an alkaloid found in several plants used in Traditional Chinese Medicine (TCM), such as Nandina domestica, Aconitum carmichaelii, and Lotus plumule. In TCM, preparations containing higenamine have historically been used to treat cardiovascular conditions, including chest pain (angina pectoris), palpitations, and even heart failure. The rationale in traditional practice is based on the observed effects of these herbs in alleviating symptoms related to "Qi stagnation" or impaired circulation. Scientific research has shown that higenamine acts as a non-selective beta-adrenergic agonist, which can cause positive inotropic (increased strength of heart contraction) and chronotropic (increased heart rate) effects. Some early clinical studies in China (mainly from the 1970s and 1980s) suggested that intravenous higenamine could alleviate angina symptoms and improve cardiac function in heart disease patients. However, these studies often had small sample sizes, limited controls, and were not widely replicated or published in high-impact journals. More recent Western research is sparse and mainly focuses on higenamine’s effects on sports performance and its presence in supplements. Therefore, while there is a traditional basis and some preliminary clinical evidence for higenamine’s use in chest pain, the scientific support is low to moderate, and it is not part of modern mainstream medical practice for angina or chest pain relief.
