Ligustrazin (also known as tetramethylpyrazine, or TMP) is an active compound extracted from the Chinese medicinal herb Ligusticum chuanxiong, commonly used in Traditional Chinese Medicine (TCM). Its use in supporting or treating chest pain, particularly angina pectoris, is well-established in Chinese medical practice and has a long history. In TCM, Ligustrazin is believed to promote blood circulation and remove blood stasis, which are considered key concepts in addressing chest pain and cardiovascular conditions. Several clinical studies from China have explored the effects of Ligustrazin on angina and other cardiovascular issues. Some have reported improvements in symptoms and electrocardiogram (ECG) findings when Ligustrazin or Ligustrazin-containing preparations were used as adjuncts to standard care. However, these studies often have methodological limitations, such as small sample sizes, lack of proper blinding, or insufficient placebo controls. Systematic reviews and meta-analyses have noted potential benefits but also highlight the need for higher-quality research to confirm efficacy and safety. Outside of East Asia, Ligustrazin is not widely recognized or used in mainstream medicine for chest pain, and robust, large-scale randomized controlled trials are lacking. In summary, the use of Ligustrazin for chest pain is primarily justified by traditional practice and supported by some low-to-moderate quality preliminary clinical evidence, but it is not yet validated by rigorous modern scientific standards.
