Ligustilides are a class of phthalide compounds found primarily in traditional Chinese medicinal herbs such as Angelica sinensis (Dong Quai) and Ligusticum chuanxiong (Chuanxiong). Their use in supporting or treating chest pain is primarily justified by traditional medicine practices. In Traditional Chinese Medicine (TCM), herbs containing ligustilides have been used for centuries to "invigorate blood circulation" and "dispel blood stasis," concepts believed to relate to cardiovascular health and, by extension, chest pain (often referred to as "Xin Tong" or "Xiong Bi" in TCM). Historical texts and modern TCM practitioners often recommend these herbs for symptoms associated with angina or other discomforts in the chest.

However, direct scientific validation of ligustilides for treating chest pain in humans is very limited. Preclinical studies suggest that ligustilides have vasodilatory and anti-inflammatory effects, which could theoretically improve coronary circulation and reduce ischemic pain. Yet, robust clinical trials in humans are lacking, and there is insufficient evidence to confirm efficacy or safety for this indication. As such, their use for chest pain remains rooted in traditional practice, with limited support from modern biomedical research.
