Nitrates are scientifically validated for the treatment of chest pain, specifically angina pectoris. Their primary mechanism of action involves the release of nitric oxide, which leads to vasodilation of blood vessels, particularly the venous system. This vasodilation decreases cardiac preload and myocardial oxygen demand, relieving the symptoms of angina. Numerous randomized controlled trials and decades of clinical use have established nitrates—such as nitroglycerin, isosorbide dinitrate, and isosorbide mononitrate—as first-line agents for acute angina attacks and for prophylaxis in patients with chronic stable angina (JACC, 2012; ESC Guidelines, 2019). Sublingual nitroglycerin provides rapid relief of chest pain and is included in major cardiology guidelines worldwide.
