Inositol nicotinate (also known as inositol hexanicotinate or "no-flush niacin") is sometimes used as a supplement to support healthy cholesterol levels. The rationale is based on its relationship to niacin (nicotinic acid), which is well established to lower LDL cholesterol and triglycerides, and raise HDL cholesterol. However, the strength of evidence specifically for inositol nicotinate is limited and weaker compared to standard niacin.

Several early studies indicated that inositol nicotinate may have mild lipid-lowering effects, possibly because it is metabolized in the body to release free niacin. However, more recent and rigorous research suggests that the conversion is slow and incomplete, resulting in much lower effective doses of free niacin. Most large clinical trials demonstrating cholesterol-lowering effects were conducted with regular niacin, not inositol nicotinate.

Nevertheless, inositol nicotinate is sometimes used as an alternative to regular niacin to avoid the "flushing" side effect, despite weaker cholesterol-lowering efficacy. Regulatory agencies in some countries do not recognize it as an effective treatment for dyslipidemia. The evidence rating of 2 reflects that there is some scientific basis (mainly by analogy to niacin and limited direct evidence), but the overall quality and quantity of evidence is low.

In summary, while there is a scientific rationale for its use, inositol nicotinate is not as well-validated or as effective as standard niacin for cholesterol management.
