Superoxide dismutase (SOD) is an antioxidant enzyme that catalyzes the dismutation of superoxide radicals into oxygen and hydrogen peroxide, thereby reducing oxidative stress. In the context of Chronic Obstructive Pulmonary Disease (COPD), oxidative stress contributes significantly to lung inflammation, tissue damage, and disease progression. Several studies have investigated the potential therapeutic role of SOD in COPD. Preclinical research and a limited number of small clinical trials have explored oral, inhaled, or injectable forms of SOD or SOD mimetics in COPD patients, suggesting some reduction in oxidative stress markers and mild improvement in inflammatory parameters. However, the clinical evidence is inconsistent and does not robustly demonstrate significant improvements in lung function, symptom control, or long-term outcomes.
Most notably, SOD is not a standard treatment for COPD in clinical guidelines, and its use is not widely adopted in clinical practice. Some dietary supplements marketed for lung health may include SOD or plant-derived mimetics, but their efficacy is not clearly established. Overall, while the scientific rationale for SOD supplementation in COPD exists due to its antioxidant properties, the current body of evidence is limited and of low quality, warranting a rating of 2 out of 5. More large-scale, high-quality clinical trials are needed to determine whether SOD has a meaningful therapeutic role in COPD management.
